Welcome to the pre-registration page for care aimed at diagnostics and indication for medical transition for adults. Before you pre-register, it is important to carefully read the information below and fill out the form completely and truthfully.
We are aware that we set many conditions and unfortunately cannot help everyone. We have chosen to be transparent to avoid confusion and disappointment as much as possible.
Important information regarding the registration process and this page:
- The registration form is posted online every two months, on the first Monday of even months at 12:00 PM (e.g. February 3, April 7, June 2, etc.). Once the maximum number of registrations is reached, the form will automatically disappear. You can find the form at the top right via the button ‘Pre-registration for gender care’
- If you are unable to fill out the form, it is not active, and you cannot register
- Registration for diagnostics and indication through a referral to regular mental health care (GGZ) is not possible at Kaleidos. We keep this care separate
- Kaleidos selects people based on the content and completeness of the registration. If you are selected, you will receive an invitation to arrange a referral through an authorized referrer (such as your general practitioner or a GZ psychologist). Your referral must be complete and received by us within two weeks of the invitation. After that, we will invite you for diagnostics and try to schedule your first appointment within a few months
- If the information is incomplete or incorrect, your referral may be rejected. We may also stop the process, even if you have already started
- Forms from people who are not selected will be deleted. We do not have our own waiting list
- Always remain on the original waiting list where you were previously referred
- You cannot derive any rights from the information on this page
Important information regarding our care:
- Limited capacity: Kaleidos has a limited number of spots available each year for diagnostics and indication for medical transition
- Requirement: You can only register if you have been on a waiting list for medical gender care for at least 2 years (e.g. at VUmc, Radboudumc, or Genderteam Zuid). Proof of this is required
- Collaboration with Gender Clinic: Kaleidos collaborates with Gender Clinic Bosch and Duin. We do not refer to other providers such as MC Bloemendaal or VUmc. Please check in advance if Gender Clinic provides the care you need via genderclinic.nl/aanbod
- No mental health care (GGZ): Kaleidos does not provide treatment for mental health issues such as trauma or depression. However, if necessary, consultations are available during the transition process. For healthcare providers who do offer intensive mental health care (GGZ), you can check the Transgender Wegwijzer from Transvisie.
- No psychological support beforehand: If you need help exploring your gender identity, you can join a peer group through the patient organization Transvisie or register for our mental health care (Registration & referral - Kaleidos). For medical gender care registration, we expect that your social transition is mostly completed (e.g., coming out and living in your desired gender)
- No switching: You cannot switch between mental health care and medical diagnostics within Kaleidos
- Non-binary care available: Non-binary care requests and mental health diagnoses are not an issue. For complex psychological or social problems (such as financial matters or housing), we refer to larger gender teams
- Expert statement: You can receive a statement from us for changing your legal name and gender, but only if you are also receiving medical gender care from us
- Guidelines and reimbursement: Kaleidos operates according to Dutch healthcare standards and international WPATH guidelines. Our care is reimbursed by health insurers and provided by a specialized team of psychologists and psychiatrists
- First appointment: During your first appointment, you will receive an explanation of how medical gender care is organized at Kaleidos
